It's been covered elsewhere multiple times, they had the wrong thread type lug nuts, they felt tight enough when they put them on, but they couldn;t hold up to the forces exerted by cornering.

It wasnt the drivers fault. He had recently purchased lugnuts from a store, and they sold him lugnuts with wrong thread pitch, so they seemingly torqued down ok, but when force was applied the sheared off.
