Just a few things I noticed, %temp% does not take you to Temporary Internet Files, it takes you to your logged in profile's temp folder which is different. Depending on the computer lower resolutions can be worse than full resolution or refresh rate and it can skip. Defrag is good but man you need to re-defrag that drive, it is messy. Once doesn't always cut it.
