It's a camera trick. the pen is laying on top of his hand while the camera is pointing upwards, it just looks like it is sideways. It is extremely difficult though.

Ballsign mod is an extremely light pen. very hard to spin but if you can you will probably progress faster and smoother than if you started with say a KT or buster.
