SiWon took the girl hostage, the girl is HanKyung's girlfriend, and HanKyung is a cop. SiWon killed HanKyung and then had heart problems. He get's a new heart, and it just happen's to be HanKyung's. Now when ever he see's the girl (hostage/HanKyung's girlfriend) he feels pain in his (or rather HanKyung's) heart.

This is my interpretation - don't find this that hard to understand.

Siwon's character is probably the leader of a band of thieves and he held the girl as hostage when confronted by the police force. Hangeng is shot within this conflict and taken to hospital whilst Siwon jumps onto the van with his companions and escape. He happens to have taken the necklace which belongs to that girl (Hangeng's girlfriend).

Siwon for some reason is in need of an operation for a new heart and happens to get Hangeng's.

Basically after that Siwon begins to get flashbacks of the incident before when he shot Hangeng, and also multiple shots of his memories of his girlfriend.

Eventually he realizes that the girl in his flashbacks is the girlfriend of Hangeng - the police who he shot, and that is why there is the part where he finds the necklace he had taken from her previously.

PS: Siwon's acting is addicting!

When Siwon passed by the flower shop, his heart starts hurting because Hangeng's heart yearns for the gf. He, for some reason, had access to police information, and thus, found out that his heart was actually the heart of Hangeng; it confirmed his suspicions since he kept on getting flashbacks of Hangeng's last moments.

Hankyung was the police officer who was ordered to catch Siwon. Hankyung found the hostage was his girlfriend and got enraged. Siwon shot Hankyung at his head and Hankyung went to coma. And he was believed unable to wake up so the family decided to give up and donate his organs to other people.
