What I understand is that he loves/misses his (probably) ex girlfriend so much that he always ends up seeing a old photo of her. He is upside down which represents his world without her and coughs blood showing how much it hurts him. The picture is always below him meaning he is forgetting her and when he shoots the real girl it doesn't symbolize he literally killed her but what is sang, you must be getting a hole in your face(pic-wise).
