Ok.. basically, the cenobites (what Pinhead is) are demons from hell (though not the traditional hell) who see pain as pleasure. Anyone with the right kindof desire can summon the cenobites with the Lament Configuration, a puzzle box. They take the person who opened it and go back to hell. Pinhead here was freed from hell after he was killed by another demon, so now he doesn't have to wait for someone to summon him.

what i do not get is at the end of hellraiser 2 which i saw the ending of, they show he was human and him and the cenobites become good only to be killed by the doctor who was now a demon, than you see him carved back in here as a killer in the next movie.

Second, he's only really a cold hearted evil guy in Hellraiser III and Bloodline, where he was made out to be the villain. His portrayal in Inferno through Hellworld is more comparable to his role in the first two, no personal agenda to rule the world and simply doing his job.

The reason he is changed,the Pinhead in Hellraiser III is purely Pinhead.After his human side(Elliot Spencer)got killed by Channard Cenobite in HR II,his human and demon part got separated. When Pinhead got resurrected only his demon side returned(its explained in the movie)Besides being no longer bound to the box he was no longer bound to the rules of his Cenobite Order. I do feel sorry for Aimee Leigh (the actress). She doesn't seem to have made much of an acting career, probably because of this scene's short length & the character she played.
