@darkchiron When you either go in the front door of the hospital or walk close enough to the front of the hospital he'll grab you. But other than that you have to talk to him. He's always at the asylum though. Worrying about asian vampires.

He can't be Caine; Caine was asleep in a cave somewhere until Beckett came and woke him up and thus initiated Gehenna.

He's probably a Malkavian Methuselah (a vampire of the 4th or 5th generation) that THINKS he is Caine.

ALso, Bloodlines acts as a prologue of sorts for the novel in which Beckett releases Caine. Gehenna comes only a few months after this game's ending.

It would be a little anti-climatic, sure, but having it as a possible ending isn't that bad. I mean, a couple of the endings weren't that good anyway. The Anarch ending was missing some scenes in the Solo ending, and the Ming Xiao ending was so so. So what's wrong with having an ending where you just run away from everything?

Hm, is he Caine... lots of arguments about this everywhere. If you're following the pen and paper game, no way, he operates on such a higher level that it would make no sense. If you read the Gehenna novel, you learn it's not chronologically possible, and Jack was hanging around with a high-generation Malkavian that just thought he was Caine. If you just look at the game, it sure seems like they're strongly hinting it's him, but nothing's for sure...

The most popular opinion is is that it's not Cain, but the Malk who thinks he is Caine.

For epicness, I myself like to think he IS cain.

If you ask me, the game devs made it Cain (look at the sound file directory, all the other hints are pretty obvious for a Vampire player) but they gave themselves enough wiggle room to say it wasn't just in case.

Yes. As far as I'm aware, the developers wanted it to be Caine, but for some reason White Wolf decided to overrule them. Fair enough, but since WW's approach to canon tends to be "do what you think is cool", I'll do just that.

It's been stated that it is NOT Caine, it was supposed to be but they wanted to take a different path with it all, so it's not caine. And that's why the sound files are still called Caine.

ok Betrams Ghoul is actually as a start around the front of the bloodbanks main entrance then after a few interactions he moves into jeanettes club. I was able to get the quests done with him as a Malk too so they are available and you can do em but complete them mainly before you leave Santa Monica the first time.

He's a vampire, but if you use that aura discipline (forgot name)you'll see that his aura is purple, every other vampire has a white-silvery aura.

It can just get annoying at the sheer amount of enemies that can be thrown at you. I believe if you alert people other enemies will just spawn out of nothing every time. I can't recall.

There is also a bugged door (as you said) in here, in the room with the waterwheel. There's a beam across the door you have to physically push aside, however it never would for me. I always no-clipped through it. Maybe it'll work this time.

That's the idea. Some of the Malkavian options cryptically say things like "You know what's going on... don't you?" and "No... it couldn't be...". It's not really helpful unless you already know who he is, however.
