its called "open throat" singing. The "Holy Trinity" (Halford, DIO, Dickinson) all do it. Halford being probably the easiest one to notice. Full diaphragm support with some adduction on the cords to get the scream happening. You cant really explain this on video. Find a good Rock voice teacher or Google "Robert Lunte"

It shouldn't hurt. If it hurts, you're over blowing the vocal folds - having too much tension on the throat muscles and using too much air pressure. It shouldn't get louder as you get higher. Singers with pure diaphragm control can sing any pitch at a consistent volume. Watch the Pavarotti master class clips on YouTube, he demonstrates that diaphragm technique flawlessly.
