I fully agree, but the issue is not of conversion alone. Its also about how much healthcare expenses wipes off ones annual earnings. I believe from my experience working in US and India (am an indian, was working in us), that if you fall really sick, or need a major dental treatment, it can easily wipe off very significant portion of your yearly earnings even if you have insurance.
Comparing the same in india, you have option of going from almost free government run hospitals(very low quality) to famous private ones(high end ones) and this does not take away huge portions from their annual earning. This makes sense as many people are still poor. But about quality, when one is in pain and dire straits, quality becomes secondary.
