the tie a button to fishing line and paste it on her head with some kind of make up, along with some blood. they pull the string, releasing the blood and tearing off the makeup.
I've never seen a convincing bullet hit done digitally. This sort of thing is the only way to go. it helps actors toom to have blood and stuff on set.

For the entrance wound on the forehead did you use a small tube connected to the larger one and concealed under the hair / hidden with makeup or is it a resivour made from derma wax and when the button is pulled the blood pours out?
