The cranberry juice in a modern Cosmopolitan is just there for the red/ pink colour it gives to the drink; Same goes with the raspberries used in the "original" Cosmopolitan.
A tiny bit of lemon/ lime is used to add a touch of freshness and counter slightly the sweetness of the orange curacao/ triple sec.
They are very similar drinks indeed; Though a bartender is more sensitive to such issues.
The more popular Cosmopolitan with vodka and cranberry juice was invented around the 1970s, though exactly who was the first bartender to think it up is disputed. This gin cosmopolitan dates back to the 1930s, and other than the name the two drinks aren't really similar at all. I prefer this gin version, personally.
