You know it might not be a too unrealistic problem that she couldn't hear herself. If you can't hear yourself, you can't stay on pitch... It's a very common live problem with singers not getting enough foldback. Unfortunately alot of the singers rectify it by copping out with pitch correction or lip-synching. Contestants don't have those luxuries. I've seen videos of failed contestants when they're actually under the right circumstances and they're not bad.
