The only problem i see though you said "you picked fish carefully" is that the one pair of clowns are picking on other fish including the other pair of clowns. Which you will if you havent already lose one of the pairs do to harassment/stress.

I'm sorry I probably should have read the description before I commented. xD I'm surprised your blue/green reef chromis is still alive.. I've had very bad luck with that fish three different times.. I had three different small schools(obviously at different times.) of 6-10 in a 55 gallon setup with canister filtration and a Skimmer.. I checked the salinity, PH, ,nitrate, nitrite, and ammonia levels once every week(All were good)with the third batch of them and they still ended up dieing anyway..

It might be fine without the volcano thing. But, if you put a figure on a rock you can see how the proportion will look wrong. The little fish suddenly become these attack of the huge little fishes. So, you see my argument now I hope. Take out the volcano island, or add more depth and area around it.
