Kurt misreads this as a hostile action, and begins to kick and hit the bouncer. The bouncer, enraged that he is being hurt for trying to help, whips Kurt onto stage and begins to try to beat him out of frustration.
by the look of it, kurt jumps into the crowd, the crowd grabs him. the bouncer does his job by trying to pull kurt back, crowd get rowdy and pulls kurt, bouncer grabs kurts hair, kurt slams guitar into bouncer's head. the bouncer then pulls kurt back onto stage and punches him. whether the hair pulling and the guitar slam were accidents or not, it looks like thats what set it off here.

I saw a different clip of this same video awhile back. The reason he punched Kurt was because Kurt smashed him in the face with the butt end of his guitar because the bouncer dude was pullin the shit out of his hair.
