@quamangcau Perhaps that the mechanical routine of driving the car would distract her from her freshly highly disturbing thoughts . . . of her conflicted loves, and her men having just given her an ultimatum to choose between them. Back then, was much less traffic and most roads were just two lane.  And few to no traffic rules, lights or signs. America was only in its second decade of having cars, and far fewer people (mostly the better off) had them.

He merely had gradually dawning suspicions that "something wasn't right," that she wanted or was having an affair w/one of those men in the car. Only after Myrtle's death, when Wilson finds the diamond dog collar and leash, does he suddenly realize that the man (as he believes) who ran down his wife had had a sordid affair w/her.
