The ending says "Got to get to you, the orphanage is closing in an hour" It could have been an accidental pregnancy through prostitution and she sent the baby there. Although what you said also makes tons of sense with "The whole world wants what we're on"

It's maintained because doors keep closing, and because everyone wants in on the action, and because anyone could be a double agent ready to gun you down. Coming clean is cutting the often times bad habit, love ofc. (but how do you know?), which is also when and why you cry.
to me i get a paranoia vibe from it. 'never expect to be sure who youre working for' and the never sleeping and one eye on the bag sounds like its about crystal or coke. but who knows
