vertical through skin.

horizontal through membrane.

vertical through membrane can result in inward collapse.

First, a vertical incision should be done to open the skin and expose the cricoid cartilage. The vertical incision limits the chances of cutting the major blood vessels around the cricoid. Then, a horizontal incision is done to cut through (Between the c-rings of the cricoid) the cartilage to gain airway access.

You can cut it either way, but like you said vertical is preferred through the tissue to avoid excessive bleeding because it's pretty vascular due to the thyroid being around it
