The disadvantage is that horses are loaded and unloaded near the car which could sometimes be in the way (dented) therefore the horse must be already trained to be loaded/ unloaded.
I had a van which was modified that horses were positioned like in your horsebox and must say it was great idea. Sometimes even veterinarians couldn't believe thair eyes about how we dare to transport horses that way but as i said before horses are much more relaxed during the drive.
