If you listen to the conversation, this is in fact a bug bite. The saliva from the critter solidified to the "gelatin" inside.

The skin was most likely removed in this case so that the wound could be properly drained and cleaned. I would assume it would then be covered by a medicated synthetic skin/cushion. I see them all the time on ulcerated heals.

This is on the heel. The area on the left of your screen where the hair gets sparse is the top of the foot. The bottom of the foot is at the top

it hurts way more with the pus/water in the blister than after you cut it open, its incredibly necessary to cut/pop it otherwise it grows bigger and hurts WAY MORE
