Really nice callback to AITF indeed, I like that it's not more emphasized than that.
I also like that in a later scene with Kovac, to explain why he looks like hell, he first finds the excuse: "I got hit by a psycho patient..." like everybody in the ER should know his history with a psycho patient should be enough to make him look not good... Except this time it's not that, and Luka is not fooled...
I don't see cracking/laughing, just breathing like getting ready to get physically violent.
