Check out PCB houses like BatchPCB and seeedstudio. Just upload your gerber files (such as made with Eagle) and they do the rest. The bargain houses tend to have low prices for 1~5 copies but they take a few weeks as they group them with other customers. You're talking $50 for five copies of a small board like this. Then you solder on your own components. Digikey or Newark are good component sources.
I have never ordered a PCB like you did there, what is the estimate cost to get a custom printed board?
