Actually the song's length is 4:44 ... and also coincidentally or purposefully 4 in some Asian country like Japan have superstitious about number 4 which is related to death or somekind of homonym to death. A fitting length for this song don't you think?

There are no rooms number 4, 104, 204 in most hospitals, it's a kind consideration for the inpatients and their families who maybe becoming nervous.
Anyway I think the length 4:44 has no meaning, it's just random :)

"Yon" is a japanese original numeral so we usually say "yon" when count something, "you-juu (40)" "yon-ko" "yon-mai" for example. "Shi" is came from a chinese pronounciation "su" for 4. We use the words "shi-gatsu (April)" "shi-tenno (4 heros)" though, there are not so much words pronounced "shi" for 4.

My grandmother was the same way...she didn't want to be placed in a hospice when her cancer got worse. she wanted to die at home with dignity and with family.
and don't forget Mai Yamane, Tim Jenson (he wrote the lyrics to a lot of her great english songs), Maaya Sakamoto, Raj Ramayya, Gabriela Robin (?), The Seatbelts, etc. :)
I think this song hits so hard because while the subject and mood are definitely sad, the acceptance of death has already happened - it's taking place in that ethereal, mysterious little gap between accepting death and death itself. It's not sad, but it's not uplifting...it's more like moving forward, cutting off the last few ties that are trying to hold it back.
It never ceases to amaze me how Kanno and the vocalists she works with can so perfectly convey all the nuances of each song's mood.

Genious, i love this song, and most of Kanno's work.
I love sad things and this one is one of my favs.
As for me i think that accepting death or accepting something sad with a calm or happy humour is even more sad.
