All I can really say is that it is a haki enhanced version of Gomu Gomu no Jet Bullet. His fist shooting flames on impact might be similar to how luffy goes vulcanization without actually going through the steps. Other words, haki related.

I can also tell you Oda needed a way to pay tribute to Ace's fire ability. This is his way of doing so.

The vapor coming from Luffy's body is his sweat boiling. Since we are warm blooded and he is pressurizing his blood to extreme amounts his skin turns redish, and he sweats because his body heats up, because his body is heating up the skin becomes hot and the sweat boils, causing the vapor. We have 2 main systems to cool our body, sweat and blood moving closer to the skin to cool, which Luffy's 2nd gear represents well with redish skin and boiling sweat causing a vapor.

He expands his bloodveins/heart, With all that blood going so fast he becomes hotter, wich make condence. Thats the smoke. Him red is blood as well. Since hes expanding his entire blood vessel/vein system. There you got your answer. All this makes him alot faster.

The reason for the smoke is the outrageous hyper increase in his metabolism... all the nutrients in his body is being burned at an extremely rapid rate increasing his body temperature making it possible to ooze smoke instead of liquid/sweat. On the part where his skin turns RED is where you are right, because of his high metabolism ... more oxygen is being distributed throughout his body causing the red blood cells to appear clearer.
