fish do feel pain, but not as much pain as we do, and not in certain areas, a different nervous system and reptilian brain (or less developed) setup.
and their not idiots, their doing alive so that it gets a better taste to the fish in the end, it requires skill to keep it alive and to get the meat, and yes they realise it feels pain.

Impartial view:
regardless of the pain caused, that fish has lost considerable muscle mass and cannot perform basic functions. I'm sure we could survive without muscles in our limbs or torso but then we could not function properly, which is the only reason I am perplexed as to why the fish was left alive.

Well, I was told by my teacher that most fish and some other marine life (with exceptions of the mammels) don't have a particular nerve that allows them to feel pain. When they get cut they just wiggle a lot because they think there is something on them.
