This happened to Shepard when I was fighting Benezia. An Asari used warp on me and it went to a cutscene, after which it put me just like that. It was extra annoying because you can't even enter the start menu if it happens to your character.

That happened to my girlfriend's game during the fight with Benezia. What happened was that she got hit by one of Benezia's biotic spells right before she summoned more monsters, so she was in the "falling down" animation. Once it returned to Shepard's control, her neck started stretching and her hands and feet were all tiny like Ashley here. Weird glitch.
