the best and fastest way to open one is to break over the back (or front) of someone's head. you won't get much to drink out of it after words but it's still opened up for you.

you need to wrap your hand around the top of the bottle, giving enough space for the lighter. make sure the bottom of the lighter is underneath the cap and open in a the normal way with a bottle opener, get a tight grip both ends and expect a little pain =)
