  • The single air traffic controller was doing "administrative duties," i.e., paperwork at the time of the crash. He didn't even bother to look up to see if the aircraft was on the right runway. Also, how did the pilots initially get on the wrong plane! To save, money the airport was woefully understaffed. I think the pilots felt that the running of the airport was so slipshod that the fact that the lights were not turned on at the runway was par for the course that morning.

  • The directional gyro and the flight director would both show the aircraft heading when positioned for takeoff. It would have shown 260 degrees if they were on the correct runway. 40 degrees is a lot of difference. I did this for 35 years. They did not see the difference. No one knows why.

  • This is what makes regional pilots what they are, and major U.S pilots what they are. Word of advice, fly out of bigger airports where there are more major U.S airlines to your destination. There are plenty of instruments in the cockpit indicating what heading your are on, how the hell do these guys not know what runway they take off from. This is embarrassing, and shows how inadequate some pilots are for their jobs.

  • This is sad and tragic and I'm sure anybody involved in this crash did not want to die, but it happened and people want to know why. Some people fault the airport, and some the pilots. I say there were a lot of bad elements the led up to this disaster. I think most of them are obvious but one that sticks in my mind the most is the runway numbering. Why are the 2 take off runways so close numerically? 22 and 26 are so close. Maybe if one of them were 35 or 17 or whatever, it may have reg

  • @Rockdog1961 Airport runways are numbered according to their magnetic heading. Runway 22 faces approximately 220 degrees magnetic, and runway 26 faces approximately 260 degrees magnetic. The runways are constructed so that they align with the prevailing winds of a certain area. In Lexington, seems like winds most often come from around 220-260, and so it works in an aircraft's favor to have appropriate runways as such. Hence, the runway numbers are very close together.

  • the pilots shouldn't take all the blame, i am a pilot and the runways are very confusing at LEX, i almost made the same mistake, only in a cessna 172, the pilots should have caught the mistake earlier, and the controller should have been watching and stopped them

  • This is the only thing I agree with you on. James was a good pilot and is a good person. My check captain at Comair trained him. He agrees that they both broke sterility policy. But, James has to live with the burden for the rest of his life and that is bad enough.

I don't think 18 people's dad didn't make it. If it's true then glad to hear. Sad still. Anytime a crash happens on a airline route, the flight number is changed. Comair will never again use 191.   • poopsy, you've got to be a retard if you believe all the crap you have been posting on the 5191 videos. Pilots were at fault for not cross checking and violating sterile cockpit rules set forth by Comair. Controller nor the airport was at fault. NOTAMs were available warning of closures. Infact, I still have the original NOTAM, as I flew out of KLEX that day myself. It was satisfactory. FAA takes the rest of the blame, as you said, for only having 1 person on duty.

  • @mchmt55116 : Understand how you feel...I'm a former gate agent with Comair. Although I didn't work there at the time, 5191 was a full flight. Your father was late to get on the flight, but someone took his place. More than likely a standbye passenger. It was a horrible accident. My heart goes out to everyone who lost their lives. This will be the 5 yr anniversary, of that flight, I just hope they remember the families and crew that lost their lives!

  • I was born in lexington kentucky. Guys its not the pilots fault it is the air traffic control. only 1 person was working. i have sympathy for the familys and feel terrible for the accident.

  • Wrong, plain and simple. It WAS the pilot's fault. If I remember correctly it was dark, and NO LIGHTS were on on the wrong runway they used. Read the CVR transcripts, these two were not payng attention at all. Glad the first officer lived, but he should never be aloud in an aircraft again.

  • It was both the pilots' and airport's fault. The pilots' because they weren't paying attention. It was the airport's fault that the control tower was understaffed.

  • The airport was not at fault by any means. The FAA staffs all fields, they set the schedules and they make sure their employees staff the facilities provided to them by the airport. The burden of understaffing falls directly on the FAA and no one else.

  • @biffwebster81 it was pilot error - the compass would have shown them they were on the wrong runway. Have mercy for the one who survived - it was human error that can happen to any of us, and the pilots were not on drugs/alcohol. It was a tragic error.

  • i was over at keenland race track right acrose the road from the airport the day before for a car show and saw the next morning on the news i wend to the airport a week later and sign the banner for the famlys

  • I had a connecting flight in Atlanta bound for Athens earlier that morning on Delta (owner of Commair) I heard the news on CNN when I arrived in Greece. Scary stuff and I sympathize with the families left behind.

  • It is my understanding that these 2 pilots routinely piloted this flight from Lexington to Atlanta. Runway 22 and 26 have different headings. I am sure the plane had a compass and worked properly. Seems logical that 1 of the 2 pilots would notice this. Albeit, I do know runway repaving altered taxi route patterns but, don't flight plans start with the initial heading? I am very sorry for families and friends of the deceased. God bless you all.

  • I don't agree. The FAA violated its own rules by allowing only one controller in the tower. That one controller had his back turned to the plane when it took off. If he had seen the plane, he could have visually recognized that it was on the wrong runway and informed the pilot. If there were 2 controllers, 1 controller could have maintained a visual on the plane until take-off. This was pilot error that could have been prevented by the controller and FAA.

  • It's my understanding (with very few hours of flight training) that when the controller clears the aircraft for takeoff, then it's out of their hand and they're no longer responsible. However, you do make valid points.

  • @cbreitel There were plenty of signs that things where not right, for example the cockpit voice recorders showed that the pilots noticed that the lights on the runway they were on where not turned on but the lights on the cross runway were turned on, that should have been more then enough of a clue to them that maybe maybe they made a mistake. This is not all the FAAs fault

  • I agree. The purpose of the video was not to place blame on any one certain person or group - just to document the event itself.

  • This was not the fault of the controller. Each pilot had a VFD that would have displayed a heading of 260 at takeoff. Also, there is a manual compass in the middle that would have displayed a heading of 260. This accident is clearly the result of pilot and co-pilot issues.

  • Plus and airport chart. Which tells the type of lighting it has. It was the pilots mistake, and my guess is that they fell into the one "Sickness" pilots can get.

  • My company lost 2 clients on that plane. I fly small planes out of lexington, no blame should be placed on the controllers. The crash was just a result of an error by the pilots.

  • IT was dark what a idiot planes take off at night more often than daylight.Bringing up the darkness is a foolish statment this a error on the pilot or air traffic controll a human error,either they were not paying attention (pilots)or traffic controll were confused.
