I know that everyone sees the Ultimecia at the top. But what about the one hanging upside down at her legs? What does that represent?

the bottom portion represents her fragmented humanity and sense of good, helpless, bound by the will of others.

in a sense the higher portion can be considered akin to a mask that shows others a monster, but that same mask is also what contributes to her pain, hence the veins binding her humanity.

the upper half protects and simultaeously smothers her inner self.

her true nature, whatever that was.

@moriel31 Actually she's absorbing everything into nothingness. Well technically she's using Time Compression to make everything fade out of existence. The "cape" here is actually the space which is fading away...
