I heard they reworked the script, the dog does die in an earlier version of the script. I don't know if they changed that with the new version, though.

In the book the dog dies. I would guess R rating with all the violence.  This is he fourth movie spawned from the book. Hollywood officially ran out of new ideas twenty years ago.
