Well, he - a man with a "black heart" - sings that the woman he's watching is in trouble, seemingly - he keeps musing about her dress and lips and stilettos - because he lusts for her. Also, because he calls her his "soft target". He sings she's in trouble, cause he's been wating for one look from her and also because "It's a long way to london bridge in them heels" which might indicate that she can't run to the safety of civilization in her high heels from him fast enough.

Wow I'never looked at it that way.

For some reason I got the impression she was a prostitute. Or she was running away from some kind of trouble but it never occurred to me that "trouble" is referring to him.

Then again I was never the best one at interpreting song lyrics.
