Ok if you watch the previous DC universe DLC you will see that lex luthor manage to kill some important superheroes (superman, flash, green lantern and wonderwoman)with the help of the other supervillains.

The reason why batman was so beat up and have cybernetic upgrade is because he took a rocket launcher fired by Joker during the fight between lex and his supervillain and bats with his team of superheroes.

Yeah, it's fully free to play and you don't have any BIG restrictions that will make you not want to play the game. But, Premium only costs 5 dollars and you can pay through you're cell phone bill or paypal/credit card. It's a one time thing. Legendary is a continuous payment though.
