It can't be aids, that disease wasn't discovered until the 1980's. I think I read on wikipedia that the writers said that there's really no real explanation for his disease.

Absolutely agree. First time I saw the ending I thought that they both died , but no..It's bigger pain to know how Kaoru-dono suffered that left time without Kenshin-san ! However,they all will live in our hearts !!! SAMURAI X LIVES FOREVER !!!
