"Gear" is slang for heroin in the UK. Johnny is on the "gear" and is a heroin addict. However, it does appear he does smoke crack as well. The movie doesn't go into specifics about what he is throwing up from, but I don't even think he is overdosing. 

BTW, you can get sick and puke from heroin, the effects are much like alcohol (even though the vomiting is normally followed by a pleasurable high when on opiates).
