  • The seat was not correctly installed. the cars eat belt was not threaded correctly. ANY child car seat that is not strapped in correctly can become a deathtrap in crashes instead of a safety device. Next time.

  • the top tether is not required in all States, Coutries etc. This can happen when a seat is not installed correctly. And I always advocate the use of the top tether. The seat muct also be installed tight and correctly each and every time
