In the UK at the manson guitar shop somewhere in Devon, they are about £119 - thats if you want one that isn't hand painted it makes no difference otherwise.

I used a fuzz factory on the following settings:

VOL: 10'oclock

GATE: 7'oclock

COMP: 3'oclock

STAB: Just move it about to get that nice scream

I had it going through my PODXT which had some minor reverb etc. And the amp used was just a Spider III
