Bankai is just the swords true form released. It vares from each individual. For Ichigo he looks like Zangetsu, for the fox faced guy it's a huge ass guy, etc, etc. And captains aren;t the only people who can use it. If memory serves Renji managed to nail it just before Rukia was rexcued

Ikkaku is as strong as a lieutenant, but does not have a limiter since he is only a third seat so he is fighting at full strength this whole battle whereas, hitsugaya, Matsumoto, and Renji are fighting at 20% until they have their limiters released.

It is the highest class of menos (hollows that are created when a hollow starts consuming other hollows) if it has part of its mask removed it becomes an arrancar which is the opposite of a vizard. The arrancar are stronger than the vasto lorde and since Ichigo is a shinigami, not a hollow he'll never be a vasto lorde. Having seen the latest manga I wouldn't underestimate old man yama or Kyouraku for that matter.
