Sorry, I meant 1790, the year Cosi premiered in Vienna--not 1789. But you get the idea--Da Ponte and Mozart wanted to stick it to Mesmerism (and give the audience a good laugh at the same time).
And a big thank you to elias12186 for uploading this wondeful production! It's fantastic.
@entropyftw This scene is Da Ponte's (and Mozart's) way of ridiculing Mesmerism--a dubious, purported "medical" treatment involving magnets that had been very popular some years earlier, before it was deemed to be a sham and fell into disfavor. It was a controversial, hotly-debated topic of the time; the 1789 Viennese audience would have understood the reference right away.
@thesir27 I believe it's not. In Mozart time, everything was probably sung in a straight-forward manner without anything as theatrical or hilarious. Some sopranos added small embellishments in their singing of fioritura, but I doubt Despina using the doctor's "old man voice" was ever heard in Mozart's time. It's a modern innovation. I love it though! I love Bartoli and Teresa Stratas who make their voices sound "elderly". It's hilarious! Look for Stratas in the Harnoncourt Cosi
@thesir27 Yes, Mozart indicated he wanted this sung "pel naso"--nasally, in other words. Back then composers worked closely with the singers and crafted the music to suit the singers' strengths and proclivities. Despina was created for the young Dorotea Bussani, the first Cherubino--so Mozart was well acquainted with her voice and buffa abilities and knew she could pull off singing comically. In real life she was married to the much older bass Francesco Bussani who sang the first Don Alfonso.
Brava Bartoli...she's a marvelous Despina...and pretending to be an old male doctor was deliciously hilarious....great job....now that's how comic opera should be done!
Se k es un papel... y se k fingio su voz... pero... ay no deeben acer esas cosas jajaja si asi es comprometerse con el personaje... k no se comprpmetan tanto jajaja, peor bueno al fin...
I've uploaded the second act and created a playlist which includes all clips from this performance (elias' first act + my second act) - see my profile.

Was the medicine that bad in society then, for them to believe?

I'm not the biggest Bartoli fan, but she sure pulls off Despina.
Gosh, so many great people in one cast! Croft is so fierce, as is Mentzer. And we surely miss Jerry Hadley!

Wow! I'm not a huge Bartoli fan most of the time, but her "Doctor" is easily the best I've ever seen! Now I want to see how she did the notary!
