London was very much influenced by the industrial revolution, and New England all along, both by immigration.
Not all humor is culturally specific. It's the comedian who makes it culturally specific. Ricky's comedy isn't things that only the English experience. Things he talks about are universal, like his childhood experiences, or fat people. That's why his Out of England version (which is exactly the same as Fame i think) was such a hit in America.
Yea they are superb. Try the following channels anydns he is the god of Ricky stuff, but try people like telepop123 and dddp666 to get a format, and get used to the idea.

What I love about Gervais is how unrepetitive he is. I know he repeated the cannibal/porn joke (there's a sentence I never thought I'd say...), but he also told the Bognor holiday story on Room 101, but the way he told it here, and ever the content, was completely different. Most comics would have a set story that they can rattle off, but Ricky is much more original.
I have seen a lot of standups, Conclusion: Ricky Gervais is number one. (And he has a lot of competition.)
