Personally I don't see it that way.

JDs about to say something, more like "Actually, I..." but is interrupted and then told by Elliot she doesn't want that (Wether she meant it or not, you can debate). JD washes that around in his head for a second then probably decided not to bring it up as it'd go in conflict with what she wants.

Long story short, I think when Ell said she wasn't up for it, that JD probably held off to keep her happy, rather than 'not having the guts'. It's how he is.

What really gives solidity to the scene [other than the music] is that hand movement/jesture Zach Braff does while staring at Elliot, representing that suppressed angst. What makes moments in shows/movies so moving is the reality of the character. It's about picking up on the little things people do and not realize that they're doing it.
