Easy way to know how low his health is, keep shooting him till he starts sprinting after you only takes about 5 knifes to finish from that point.

I got lucky and got this on the first juggernaut I fought. For me at least, the trick was to empty a fully loaded Model 1887 into him and knife him once (it was a last-ditch effort to survive, and it just happened to be successful). I was playing on Underground, which is one of the maps designated "easy", so I don't know if this had any effect on his overall health, but hopefully this will help some folks.
