I have seen this problem during my many years of flying the DC9/MD80 series. Most pilots visually check the flap/slat setting several times before takeoff. Some airlines teach the technique of pushing the throttles forward before take off to check for a configuration horn, and in this situation, they wouldn't get a warning horn because the airplane already thought it was in the air. Very sad situation.
The original problem was that the nose strut was over-serviced and that switched the aircraft systems into "air" mode. The reason for the delay was that the Thrust Rating Indicator was showing an erroneous power setting because the Ram Air Temperature gauge was receiving heat (it only receives heat when in the air). The mechanic put the TRI on maintenance deferral without realizing the airplane was in air mode, and the take off configuration warning (flaps warning) only works in ground mode.
This is a real video taken at Barajas airport in Madrid, the reason for the accident was the desconnection of the flaps alarm, this caused a stall on the take off and the final crash on a small river near the ariport. Many casualties.
The investigation showed that the pilots attempted to take off with flaps and slats set to 0 degrees. They apparently rushed through the checklists and didn't actually CHECK, resulting in the death of nearly every person on board. This was a clear case of involuntary manslaughter on the parts of both the pilot and the co-pilot. Fortunately, they gave themselves a self-inflicted death penalty, saving the need for a trial. Criticizing the airport design because, a good four or five hundred feet beyond the runway there is an embankment is no different to complaining that it a mountain is there when a plane crashes into it. Sorry, but I don't agree with you. Mountains are not man-made, airports are. The bad design of the airport lanes is obviously not the direct cause of the crash, but it worsened the effect of the crash since it difficulted the rescue of the victims. Cities sorrounded by mountains have always had troublesome airports, but this is not the case. They should have preview that the embankent was way too close to the track. It would have not prevented the crash, but It could have saved lives.
@UHF43 All I mean is, how far do you take it? You can't cater for every eventuality. As long as the airport is safe for the extremes of take off and landing, that is sufficient. Anything outside those limits, and there's likely to be a disaster whatever happens. When you look at the speed this plane was travelling at when it left the runway, I can't imagine there'd have been much less of a disaster had the embankment not been there. If it'd stayed in one piece I'd have been very surprised.

@tircalina Eso es porque el avión explotó al salirse de la pista y caer por el terraplén hasta un arroyo al que era casi imposible acceder. Sorprende que se permita construir un aeropuerto con semejantes alrededores agrestes, porque no hablamos de un avión que se cae a 2 kms del aeropuerto, sino de uno que se sale de pista y a menos de 200 m tiene un terraplén enorme que tuvo gran influencia en el fatal desenlace.
igual que hay accidentes de coches, los hay de aviones. Lo que me sorprende es que no hayan mas accidentes dado que los aviones tienen una cantidad increible de aparatos y dispositivos indispensables para el vuelo no pueden fallar. Please shut up. The CLB MODE was activated during takeoff, which caused the error. In the cockpit recorder you can listen to the flaps checklist before takeoff. The problem was that the flaps didn't work and with CLM MODE on, the alarms didn't call off.

Hang on a second here amigo. Did the pilots extend the flaps or did they not extent the flaps? So there is a mistake then between man and machine. There was a breakdown somewhere along the chain of command and therefore the flaps of the MD82 failed to extend on takeoff. End of story. The pilots failed at the end of the
