I think the mental hospital and the US government are both involved in this mess.

The treatment of mental patients in mental hospitals is just like what happens in the prison. If the patient is not mental but posses a threat to them, they will be deemed mental, if they are not in fit condition to stand in trial physically which also impacts their mental health the jury will declare she is fine and fit.
