  • Just got diagnosed with piriformis, don't have time to get PT but this will surely help. Thanks for sharing - no one understands just how painful it is until they actually have it. Husband is starting to develop it too, due to long commute in the car to work. I will teach him these exercises. Can't wait to see how this works!

  • I find elliptical machine, flares up Fibromyalgia pain in the hip muscles and hamstrings, no matter what resistance or elevation!  DO NOT DO ELLIPTICAL WITH FIBROMALGIA AND PIRIFORMIS SYNDROME!!!!! I know as a Therapist and from experience!

  • I hope this works! I've worked so hard to go from non-runner to running 6.1 miles and now I can't even walk fast. The weird thing is I can do the elliptical w/o pain. I can't take any anti-inflammatorys , so all I can do is stretch. I am praying these work. Ibtw - I love how you said eternity at the end. ;)

  • @misssdmj Same issue here. Also do the elliptical w/out pain. In addition to these exercises, I find it helps to do the elliptical in both directions, 20 minutes each direction. Best of luck

  • I just want to say to some of the people watching the video, this is not for sciatica. it is different. it is for Piriformis Syndrome. It involves the same nerve but is a different feeling and problem alltogether. I've had both, and can tell the difference. He is suggesting these stretches for Piriformis Syndrome. If you try them for sciatica without investigating further, you may be hurting yourself

  • This is fantastic. My ortho said he suspected piriformis but was hesitant to diagnose without a very expensive scan. After only a day and a half of these exercises I feel a tremendous difference. THANK YOU!

    I have a bump on the back of my hip about an inch from my spine. Do you think this is a knot causing the issues? Or something unrelated? It didn't show up in my xrays.

  • It is true that the head and shoulders when stretched away from the hips positively affect the pelvis area and the Piriformis muscles. Good video.

  • This is one mazing video. I self diagnosed Piriformis having spent too many hours sat down travelling for work. I tried these exercises and the pain was instantly reduced. I had pain in both buttocks so this is a massive relief.

  • Technically, that's neural stretching. Neural flossing is slumped forward, then sitting up straight and looking up as you straighten the leg.

  • This is very helpful-I described my symptoms to a relative who is a choipractor-he suspected piriformis-your description of the symptoms are spot on. I have started the stretches and it helps! Mine is on the righ leg-driving is extremely painful so I am looking forward to this working!

  • Thank you SO much for this! I am crippled from it. I can only walk for two hours a day. Had spine shots & chiropractor once or twice a week. Accupuncture, iced it, heated it, eat a clean diet and yet no one ever told me about this. Did the first exercise twice (barely) and nearly threw up, like after running a marathon. Had to lay down but I know now what the problem really is and that it can be fixed. Thanks SO much. Your a life saver! Was contemplating seeing a surgon next week. God Bless You!

  • I've noticed I'm a lot tighter, sometimes achy, if I sleep with my feet out from under the covers. Gotta make sure your whole body is warm while sleeping. You'll wake much more limber, have to do less stretching to regain best flexibility. Also, how often you stretch has a lot to do with how limber you maintain. 3X / day keeps me able to put my head to my knees straight legged at any moment.

  • I don't know what prompted you to put this on YouTube while in a hotel but I will be forever grateful! Your video explained to me what my chronic problem was and how to treat it. Something a number of visits to 2 physios, 2 chiropracters and a medical doctor had failed to do over 3 years or more. Since carrying out the excercises you recommended plus "Glutes- Piriformis Muscle Stretch" I have had great improvement - reduced pain and a restored feeling of strength to my leg movement.   • this won't work, this will only make the problem worst, thats why if you dont do this for 1 day it will go really bad

  • @Bosskane why do you say this will make the problem worse? I recently hurt myself performing single leg RDLs and my pain has been diminishing for a few days but the numbness and pain during walking or any isolated leg movement is still severe. Have you any other suggestions to help this besides the obvious? (core and back strength)

  • I have lower hernaited disks & some other related issues & I found the best place, for myself, was to do these types of streches,especially the second one you showed, in a pool. I just lean against the pool & stretch. The water just allows me to move more freely because I have a great deal of pain to begin with. Thanks for the video

  • Thanks for this video Brett. My name is Patrick. Im in fresno ca. I've been streching my muscle with similar streches that you showed for months. But i'v never streached my SI nerve before.I'v been able to walk with no pain, but I need to run becuase im trying to get into the police academy. For months now, no progress. I couldn't run 30 feet with out really bad pain. I strechted last night using your technique and I went for a mile run, no pain. Thanks for everything. Your my HERO !!

  • Heat on ankles in extreme cases can help.

    In my case, the sciatica was too painful to do these excersice. Using a large back vibrator, I found some very painful spots on my feet and ankles.  A little research revealed the the sciatic nerve goes through the outer-ankle and then branches out in the foot. A hot water bottle or microwavable bean-bag on the outer ankle can reduce (eventually) the pain enough to do these stretches.

  • This is a good video. Most people do not understand how terribly painful this can be. My first encounter with it was when I pulled the Piriformis in my left leg (im left handed if that makes more sense) and that was pretty much the worst pain I ever had in my life. I couldn't walk more than 50 feet without squatting down and stretching until the pain went away.

  • Thank you for this video. I've had x-rays, and MIR's for this pain. I was told I was developing OA and just had to live with it. The doctor did not consider soft tissue problems and I had trouble describing the pain because it was referred and felt like leg and back problems. This pain leads to depression as I was wondering how I was going to work until age 66 (I am 57). I self-referred to another doctor after more than a year with pain. He sent me to PT and piriformis syndrome is being treated.

  • I have had piriformis/sciatica on and off for many years

    I have tried many different stretches......most were mildly effective but no real relief.

    I did one set of these exercises and the pain was gone!!!!!

    I believe it is the "neural flossing" that is making the difference.

  • The info is fine as far as it goes. However, the piriformis can compress the Suprerior Glute nerve as well as the Sciatic and cause walking pain and limp. This is known as the Trendelenberg Gait. I know this because I had this problem until I had a Piriformis release.

  • THANK YOU!!!! I saw this video a few months ago and forgot your comment about doing these daily or the pain will come back. Well, the pain came back! Now I find the additional comments about keeping my feet parallel. I realize that the trouble began when I started siting at my desk with my feet pointed to the sides. I was a dancer and know the importance of keeping parallel when moving. It never occurred to me that I need to do the same while sitting! Thanks again for the video & comments!

He gave me a shot and said: It will be better in a few weeks. But it wasn't. I guess, next time I'm coming to you instead of him. ;-) Best wishes.

  • Wow! I have been going to the chiropractor for 4 months trying to help this. My massage therapist suggested I google piriformis syndrome and see if the exercises would help. This helped me the very first day I did it. It isn't totally gone but SOOO much better! I am not a runner but want to start couch potato to 5k. Walking my usual 3 miles had become an exercise in pain tolerance and now I am so hopeful that I will be able to do begin 5k training soon. Thanks so much!!!

  • 3 months ago I went on 6 mile jogs, now I no longer work and spend over 23 hours a day in bed because standing up is so painful. I'm going to try these exercises and let you know how I get on.

  • Thank you, thank you, thank you! I've been suffering from piriformis syndrome for months. Now in less than 24 hours after doing these simple stretches, I'm almost cured! I thought I was going to be an invalid for the rest of my life! I think mine was caused by unbalanced exercises. I will do more stretching and less contracting (tucking butt) from now on. (by the way, Brett, you could be a twin of my second son - that makes you even more special :-)

  • Great tips, but the first exercise you do stretches the nerves all the way through the spine. While this is fine, you should NEVER push into pain with a nerve stretch - they are very different from stretching muscles - you can cause lasting damage over stretching a nerve. You want to aim to be able to raise your leg to a 90deg angle in a slouched position. However, for me I feel pain from simply placing my chin on my chest in a slouched seated position. Over time I can work toward this exercise

  • ... sitting or laying down. While driving or at a desk make sure the bad leg's knees and toes are pointed forward instead of to the side. When sleeping, don't let the leg flop over and point to the side. Shove a towel between the car door and your knee to keep it straight. Sleep on your side instead of your back. This seems to stop the CAUSE while the video is more of a temporary band-aid. Keep up the hard work and remember - THIS IS CURABLE!!!

Question: how long do you do the second one (sitting, crossing leg over other leg), and how long do you do the third one (leg up on wall at 90 degree angle)?

  • Comment from a massage therapist...why does the muscle keep tightening up when you stop the exercises? One possible cause/contributing factor might be your posture. Your piriformis laterally rotates your leg. One easy way to check whether your are too laterally rotated is to look at your feet when you're sitting or standing. If your feet/foot are pointing out to the sides, making small changes in the way you walk or sit should help. Think about parallel-ish rather than a big V.

  • i have final honors exams in two days and iv got this pain for the past almost a year, it wasnt so much so i never gave it much attention but now that i have exams i am sitting more that ever as i need to study and had a dissertation and now the pain has become unbearable, i couldnt study and my papers are each for 3 hours i couldnt imagine siting for so long with this pain !! iv just done the exercise a few times now and i feel a little better already!! thanks alot you really saved me!!

  • Being aware of how you may be facilitating an imbalance in your posture can be one step on the way to getting improvement. For example, when you stand,do you tend to lean on one side more than the other? Are always leading with the same foot when you start to walk or climb stairs? When you are washing dishes, brushing your teeth, sitting...are you leaning more on one side?.  Along with the exercises, paying attention to movement and trying for evenness and balance has helped me.

  • Check out Dr. Hal Martin, he operates piriformis arthroscopically and has great success rate compared to previous methods. I am trying to get the word out and hope other sufferers do also so this becomes more main stream!!!! Tell your doctors, look at the improvements with surgery in only the past 4 years, lets keep it up!

  • I suffer from Piriformis Syndrome and even though this does not FIX it, it somewhat DOES RELIEVE the pain . Thanks for the upload. Piriformis Syndrome is such an annoying constant pain that drives you insane. Sometimes I wish I could just chop my butt off. So...needless to say, I'M GRATEFUL FOR YOUR ADVICE.

  • Neuro flossing is where you create tension on one side releasing it on the other side. Therefore flossing the nerve and hopefully breaking any adhesions in the neural sheath. This was a nerve stretch not nerve flossing. And this is just one technique. I wouldn't say it will fix it it depends on the person and their level of piriformis syndrome

  • The stretches worked, helped me sleep, but the pain came back pretty strong.

    After a couple days I tried muscle building supplements from the health food store (with the stretches). Dramatic relief overnight. I did DHEA +Super Miraforte +Sarsaparilla for testosterone and Glutamine (2gm 2X/day) for GH. I'm virtually pain-free.

    I'm older, 50+, so help for my hormones makes sense, but I recovered faster this time than when I had the same problem years ago and did similar stretches.

  • Yeah, I agree he sounds "goofy" but what he is saying is golden. I did this several years ago (minus the feet on the wall) and it fixed mine. Now i do it about 2 times a day about 4 days a week along with sciatica exercises and they have worked for years...

I have a herniated disc & am very cautious to try things....but the neuro-flossing really helped me! Your other exercises have helped me too. My chiropractor also cautions that I use heat on my muscles & ice on my spine after stretching, which has also helped me. Thanks again!

    Another stretch that really helps me is similar to the last two things except you sit upright with your painful leg folded under in front of you, leaning forward, using the ground to help you stretch instead of the pressure from your other leg. This can be nicer than lying down, since sometimes it hurts to get back up from a laying position when my sciatic pain is really bad.

Went through painful and unnecessary process of getting an epidural because I was told pain was related to a bulging disc which did absolutely nothing to ease pain and as a result I'm left with tons of bills. What a load of crap! Just doing these stretches and already I'm feeling tremendous relief.

  • I got Piriformis Syndrome by over-use of a NordicTrack ski machine. It's great to see such sensible, concise advice from a fellow sufferer. A suggestion I would make for the last exercise is that you can be even more comfortable if you rest your right calf on a chair or low stool. To get the best stretch, grip your left heel with your right hand and turn your left foot "outwards". Finally, gently wiggle your left big toe. All this, of course, is for left sided pain.

  • Step 1 hurts man!!!! I suffer piriformis on both sides it has been crushing my sciatic nerves for over 5 yr's now, I have never felt releif as instant as this thank you

  • Piriformis Syndrome is a major league pain in the butt!!! I didn't stretch after a few long runs and now I'm paying for it with a recurrence of butt pain. You are so right that these have to be done, basically, forever. Thanks for sharing those stretches!!

  • rather than tension a glide would be better in regards to the first exercise...less stressfull for the nerve

  • Thanks sooooo much for these stretches! I have recurrent periformis syndrome & I will do these stretches and keep doing them forever, as I am in a women's bowling league in fall and winter, golf alot in summer, garden and am very active, so I never give this muscle a rest, which is why I think I keep aggravating it. I'll try this regimen you showed and let you know if it worked! Patty

  • thanks very much for that....already feeling better. My Piriformis has been intermittent, but servere at times. My dad insisted I should ice it -- BIG mistake. Heat packs loosen up the cramp somewhat. Now doe that video again with no skivies on.
