If you look at 1:23 you will see there's an officer with a horse on the side but he's not on it he's just holding the rings that's the horse that got loose no one fell off there horse. The horse didn't want to be left behind.

Oh I feel sorry for the rider, falling on pavement would HURT. I wonder who got hold of the horse in the end. I think it's more of a symbol that the spirit of Diana is there to show her approval of the wedding. Diana runs free and wild now, no longer hounded by the paparazzi and happy that her son is happy in love.
