Some of the people in Iran who obtain GRS are transsexual. Some of them would be non-op in the U.S. and some would actually identify as (fem) gay or (butch) lesbian if in the US, but who will submit to surgery to avoid the severe penalties for being found to be homosexual. At least for those who want the surgery, Iran provides an avenue, unlike other Islamic countries. It is a *very* different culture.
