it`s kinda difficult to explain what Todo Cagado means in English, because the word "todo" is used, as a slang. You see, todo means "all, everything" but when its written together, with the word cagado it changes the meaning to something like "completely", "Entirely" crapped. (in the song, "cagado" is pretty close to fucked also)

They are meant to be heard separately. But in the beginning of the song, the first word "Todo" (indefinite pronoun, adjective, singular, masculine) derives from "tudo" which means all/everything. And "todo" together with any other adjective it's just to give more emphasis. I think it is the same in English.
