While training with Jiraiya, he only went up to Three-Tails because his seal was loosened. So naturally, the Kyuubi's chakra leaked out, but since it couldn't take control of Naruto's anger because he had none, Naruto had control.

When he went against Orochimaru he was again in control even up to 3 tails, but again, really pissed. In control in this case means he knows who are friends and who are enemies, which he can no longer differentiate when he reaches 4 tails, culminating in him attacking Sakura.
