A doctor proposed, doing head surgery, Cao Cao was enraged at this proposal and executed the doctor, wondered how the head surgery would've turned out if Cao Cao accepted....

He starts with the Yellow Turban Rebellion and Hu Lao Gate with everyone else and follows Cao Cao around talking with him. Then he has Guan Du, Chang Ban, Chi Bi, and He Fei where he fights against Cao Cao. Lastly, Yi Ling and Wu Zhang Plains where he fights for Shu's side. As for his story, it was mostly him just trying to gauge everyone's ambitions and trying to find out who he thought should rule the country.
