You don't do anything. You just open up the program, and it does all the work for you. But yes, it opens up valvecafepc##### and you have all games, VAC2 unbannable, server unbannable.

Cafe is NOT a patch. Steam directly gives you access to it's Vavle ect games. And as you can see, I downloaded like 10 there, and actually started a downloaded in there. So no, you don't have to download it first, then run cafe.
