Personally i think it has to do with the Second Coming of Christ. You will notice that it refers to the 23rd chapter of Matthew that gives the parable of the 10 virgins, which is about the second coming. So in a sense it is about Christ and his coming. Death, Armaggedon is all secondary, but still an intrical part of all of it.
the song IS about Death as an entity. note at the end where it says "And i heard a voice in the mist of the four beasts, and I looked and behold, a pale horse, and his name who sat on him, was Death, and hell fought with him."
This is the Revelation and the coming of The Four Horseman. The song is about Armageddon. There is no Rapture. Only Death, Suffering, the Antichrist, and the Second Coming of the one known as Jesus.

Of course his coming and judgement are heralded by the end of the world which of course includes a lot of death. However you make it sound as if death is an entity which as far as the context of this goes is not so.
That said, Scar=Jesus doesn't quite work either. Especially since frankly FMA is fairly friendly to Christianity in my view at least. Still good video.
If you want to debate the meaning of this song, go and do so on another video. This was a good choice, mate, I think it fits Scar perfectly.
